Home Telehealth partners with Veterans to improve their Health!


Mission:

The Home Telehealth program was developed to help you manage your health care needs in the comfort of your home. Using in home technology, RN Care Coordinators are able to monitor and assess your health and well-being.
The RN Care Coordinator serves as a link between you and your PACT Team.

Eligibility:

1. You must have a VHA Provider.
2. Have a chronic illness such as:
  •      Congestive Heart Failure
  •      Chronic Obstructive Pulmonary Disease
  •      Diabetes
  •      Hypertension
  •      Depression
  •      Anxiety
  •      PTSD
  •      or other chronic illnesses
Benefits:


  • Motivates you to actively participate in your own health care
  • You will have follow up phone call for your symptoms and treatment plan progress as needed
  • You will learn to better manage your illness or condition
  • You will be able to receive e health information in your home (all training will be provided)
  • You will benefit from more timely help to avoid hospitalization or ER visits.
